The Department for Education will only allow suitable persons to establish and run publicly funded free schools.

This form is for free school applicants. Each current and proposed member and trustee of the academy trust must complete the form separately.

Make sure you fill in all the personal information required on the form. This is so that we can check your suitability to manage or run a free school.
